Planning a shopping trip
Discuss making a list, working within a budget and the practical help you want when shopping.
Support for a life that feels like yours, at home and out in your community.

Our community outreach service supports adults, including younger adults, who need help with everyday life and accessing their community. We start with what you want to achieve and work at a pace that respects your confidence and choices.
Support may include shopping, budgeting, appointments, meal preparation, daily routines, social activities and practising independent living skills. The activities and level of assistance are agreed through an individual plan.
We welcome enquiries involving learning disabilities, autism and mental health needs. We assess communication preferences, sensory needs, risks and staff skills, and discuss any involvement of families or professionals before agreeing a package.
Your goals might be attending a regular activity, preparing a meal, keeping an appointment or feeling more comfortable in the community. Reviews consider what is working for you and what should change.
We agree goals with you, then review whether the support is helping in everyday life. Here are some of the areas we can discuss during your assessment.
Discuss making a list, working within a budget and the practical help you want when shopping.
Explore the steps you want to practise and the prompts or assistance that suit you.
Talk about your interests, travel arrangements and what would help you feel comfortable taking part.
Agree how you would like support with planning, getting there and communicating your preferences.
We discuss the goal, the help you want, any risks or practical arrangements, and how you would like progress to be reviewed. Your preferences remain central to the plan.
Tell us if written prompts, a familiar routine, a quieter environment or a particular way of communicating helps you. We consider your preferences alongside the staff skills and support arrangements needed to meet your needs.
If personal care is also needed, we discuss and assess that separately within the appropriate care arrangement. This service page describes adult support and does not offer supported accommodation or housing.
Tell us which service you are interested in and where you need support. We will discuss your needs, assessment arrangements and next steps.
