Returning home after hospital
We welcome hospital discharge and reablement enquiries. The assessment, information available, staff skills and current capacity determine whether and when we can support a safe return home.
We discuss visit length, timing, staff introductions, cover arrangements and the people you want involved in reviews. We confirm the support we can provide before care begins.
