You can use this guide to prepare for a conversation with us, on your own or with someone you trust. You do not need to have every answer. The assessment is an opportunity to explore your needs and preferences together.
What matters in your day?
Think about your routines, interests, culture, food preferences and the things you would like to keep doing for yourself.
Where would support help?
Consider daily life at home, getting out into the community, companionship or a return home after hospital. Tell us what feels manageable and what has become difficult.
How do you like to communicate?
Tell us about your preferred language, communication methods and any adjustments you would like us to consider.
Who would you like involved?
You choose who takes part in discussions and reviews. We will talk about consent, privacy and how information should be shared.
What practical arrangements do you need?
Ask about proposed visit times, staff introductions, cover arrangements, contact details and how a change or concern will be handled.
What will your care cost?
Ask for a written quotation covering the agreed support, any additional charges, payment arrangements and service terms before you decide.
How will your support be reviewed?
Discuss your goals, how you will share feedback and how the plan can change as your needs or preferences change.
A brief overview, postcode area and your contact details are enough to start a conversation. Please do not include medical records or detailed personal care information in the website enquiry. We will agree a suitable channel for that information.
